This precedent-setting contract is the first of its kind in the country and is being used as a model for other states to follow.
Over two years ago the HealthSpan medical team started recommending the YMCA’s Diabetes Prevention Program to their prediabetic patients. HealthSpan (formerly Kaiser Permanente) medical leadership and physicians have had a long history of being proactive in keeping their patients healthy, rather than just treating them after they were ill.
The YMCA’s Diabetes Prevention Program gave them a proven evidenced-based program to help prevent or delay the onset of type 2 diabetes. Over the past two years, the reach of the program has grown to all of the HealthSpan medical offices in the Greater Cleveland area and surrounding counties. The YMCA of Greater Cleveland Diabetes Prevention Program Director holds monthly on-site meetings with referred patients at two HealthSpan facilities, and the Cleveland Heights Medical Center also hosts a number of Diabetes Prevention Program year-long classes.
The state-wide HealthSpan contract and coordinated referral system is the first of its kind in the United States. Never before has a YMCA contract been written for a Healthy Living program with an integrated medical and insurance provider.
